Meis’ television career began in 2003 when she participated in the casting of moderators of the television channel TMF. She later hosted Fox Kids. In the beach soap Costa!, she played a waitress for two years. She also hosted the annual TMF Awards and interviewed international artists such as Kylie Minogue and Britney Spears. She has also appeared in music videos and created her jewelry collection Pure de Sylvie, which was launched in Germany in 2006.

Meis and her then-husband moved to Germany after he left Ajax to play for the Bundesliga club Hamburger SV. When her husband moved to Real Madrid in 2008, van der Vaarts moved to Spain. From 2008 to 2011, she was a judge on the German version of Simon Cowell’s Got Talent, Das Supertalent. In 2010, she participated in the third season of Let’s Dance, the German equivalent of Strictly Come Dancing. In 2003, she began dating then-Ajax footballer Rafael van der Vaart. Because of their fame from their careers, they soon became the Netherlands’ most famous “football couple”. On 10th June 2005, the couple got married and broadcasted live coverage of the wedding on SBS6, a Dutch television channel, with the broadcast gaining high ratings in the Netherlands. On 28 May 2006, the couple welcomed their son Damián Rafael.
In 2010, Meis admitted to cheating on her husband, saying van der Vaart discovered via email that she was having an affair with a KLM airline pilot, which she believed was the beginning of the end of their marriage. On January 2, 2013, Meis admitted to their breakup after slapping her in front of their guests. Their divorce was finalized in December 2013.
On April 12, 2017, she got engaged to Charbel Aouad, but the couple split in October 2017. On 19th September 2020, she got married to German contemporary artist Niclas Castello in Florence, Italy.

On June 16, 2009, Meis announced that she had been diagnosed with breast cancer and underwent surgery in May 2009. She completed post-surgery chemotherapy and has been cancer-free since then.
